Rhythm Identification and Interpretation

Rhythm interpretation is the largest single domain on the Certified EKG Technician exam, roughly 35% of the questions, and it is the part of the job where recognition speed genuinely matters to a patient. This chapter builds the skill in the order the exam tests it: first the waveforms and the numbers that define normal, then the three ways to calculate a heart rate, then a rhythm-by-rhythm reference covering sinus, atrial, junctional, ventricular, blocked, and paced rhythms. It closes with the two judgment calls a technician makes every shift, telling artifact from a real rhythm and knowing which rhythms require someone at the bedside immediately.

EKG Acquisition, Lead Placement, and Troubleshooting

About 28% of the exam covers the mechanics of actually obtaining a tracing: where each electrode goes, how the machine is calibrated, how the patient is prepared and positioned, and how to recognize and fix the artifacts that make a tracing unreadable. This is the domain where an inch of misplacement changes what the physician sees, so the anatomic landmarks are worth memorizing exactly. Everything here assumes the standard settings of 25 mm/sec paper speed and 10 mm/mV standardization unless a deliberate, documented change is made.

Safety, Communication, and Professional Responsibilities

Roughly 19% of the exam covers everything surrounding the tracing itself: identifying the right patient, protecting privacy, controlling infection, staying inside your scope of practice, responding to emergencies, documenting accurately, and preparing patients for Holter, telemetry, and stress testing. These questions are usually scenario-based, and the correct answer almost always follows the same logic: protect the patient first, report to the licensed provider, and never act beyond your training.

Cardiac Anatomy, Physiology, and Common Conditions

About 18% of the exam tests the anatomy and physiology behind the tracing. This is the smallest domain but the one that makes every other domain make sense: chest lead placement follows the position of the heart in the chest, escape rhythms follow the inherent rates of the conduction system, and the ischemic changes on a 12-lead follow the coronary anatomy. This chapter covers the chambers and valves, the path of blood, the conduction system and its intrinsic rates, the mechanics of the cardiac cycle, and the common cardiac conditions a technician will see referenced on requisitions every day.
