DEA Form 222, or an electronic order placed through the Controlled Substance Ordering System (CSOS), is required for every purchase or transfer of a Schedule II substance between registrants. Theft reporting uses Form 106, destruction uses Form 41, and initial pharmacy registration uses Form 224.
