Medicine
The Medicine section is a catch-all covering immunizations, injections and infusions, psychiatry, ophthalmology, cardiovascular services, physical medicine, and more. Because the services are so varied, the exam focuses on the reporting rules that tie them together — pairing products with administration, combining E/M with add-on services, and unbundling distinct procedures correctly.
Vaccines, injections, and administration codes
Immunizations require two codes: the vaccine/toxoid PRODUCT and the ADMINISTRATION (route and, for younger patients, whether counseling was provided). Reporting only one undercodes the encounter. Therapeutic and diagnostic injections/infusions similarly separate the drug (often a HCPCS Level II 'J' code) from the administration service, and hierarchy rules determine which administration is 'initial' versus 'subsequent/sequential' when several are given.
Combining services and unbundling correctly
When a psychiatrist provides both an E/M and psychotherapy in one encounter, the E/M is leveled by MDM (not time, since psychotherapy time is carved out) and the psychotherapy add-on is chosen by its own documented time. When NCCI edits bundle two services that were genuinely distinct — separate site, session, or organ system — modifier -59 (or a more specific X{EPSU} modifier) may unbundle them, but only with documentation to support it. Modifier -59 should never be used solely to defeat an edit for payment.
