Section 1632(b)(1) says floor, roof and skylight openings shall be guarded by either temporary railings and toeboards or by covers, and personal fall arrest is available where the work makes a rail impossible. What makes (c) wrong is exactly what makes it common: section 1632(b)(3) requires a cover to support the greater of 400 pounds or twice the weight imposed on any square foot, to be SECURED in place against accidental removal or displacement, and to be marked with a painted or stencilled sign reading Opening - Do Not Remove, in letters at least one inch high. Loose plywood satisfies none of that and slides. A sign (a) and caution tape (b) carry no load at all; the standard never treats a visual warning as guarding.
